3. Clear Photocopy of Baptismal Certificate or Dedication Certificate for Non-Catholics

4. Duly Filled-out Student Health Record Card [kindly get the form at the USC-BED, SC Guidance Office]or download and print the form by clicking SCHOOL HEALTH RECORD CARD

5. Stool Laboratory Test Result [ for prep – grade three applicants only]

6. CBC Laboratory Test Result [ for grades four-six applicants only]

7. Chest X-Ray Result [for High School Applicants only]

8. One piece 2x2 picture of the applicant’s parent or guardian

9. Admission Test Result

10. Recommendation Note [Request for it at USC-BED, SC Guidance Office]
